Accessibility and Disability Services Office contact information

All disability services advisors can be contacted via email and telephone.

Accessibility and Disability Services works with all University of Phoenix students. Please call or email our Disability Services Advisors with general questions or for guidance throughout the accommodations application process. When contacting us, please be prepared to discuss your need for accommodations and how you plan to study with the university.

Complaints and grievances:

If an issue arises regarding your accommodation, delivery of accommodations in a course, the classroom environment, as related to your disability or accommodations, other challenges or difficulties related to your disability, please consult your Disability Services Advisor, a member of the Disability Services Management Team or the Section 504 Coordinator.  Students who wish to file a formal ADA grievance must inform Accessibility and Disability Services and complete the Initiation of Formal ADA Grievance Form.  Students may also file a complaint directly with the Office for Civil Rights in the U.S. Department of Education regardless of whether they have filed an internal grievance.
